White Icon, No Name, No Zoopedia...help?

I made a new plant with Nique's Foliage Design Tutorial, and followed it as best I could, but after checking and rechecking my files, I cannot figure out why I have a white sqaure for an icon, no name and no zoopedia article, HELP!!! I AM SOOOOOOOOO CLOSE TO MAKE MY FIRST PLANT!!!!!! (Note: This was my eighth try)
